Jack P. Kotlarz, M.D.
Dual Board Certified Doctor of Medicine
Dr. Jack Kotlarz specializes in the practice of Ear Nose and Throat problems and Facial Plastic Surgery. He attended Syracuse University, majoring in biology and psychology graduating summa cum laude. After serving in the Army for three years he attended medical school at the University of Illinois in Chicago. He finished his residency in Otolaryngology at the University of Alabama in Birmingham, Alabama. He then completed a fellowship in Facial Plastic and Reconstructive Surgery in Sacramento, California. Prior to locating to Pensacola, Dr. Kotlarz was an assistant professor at the State University of New York at Buffalo, New York. He maintains hospital privileges at Sacred Heart Hospital, Baptist Hospital and West Florida Hospital. He is board certified in both Otolaryngology and Facial Plastic Surgery. He is currently on staff at the Florida State University College of Medicine.
He has lectured nationally and internationally on topics related to ENT, cosmetic and reconstructive surgery. Dr. Kotlarz routinely volunteers his services at many medical missions providing care to individuals in developing countries.
Dr. Kotlarz is a member of the American Medical Association, the American Academy of Otolaryngology Head and Neck Surgery, the American Academy of Facial Plastic and Reconstructive Surgery, Florida Medical Association, and the Escambia County Medical Association (past president).
Dr. James Wold, PhD
Board Certified Doctor of Audiology
Dr. James Wold has had many years of experience in the field of speech and hearing rehabilitation. Dr. Wold recently retired from the Pensacola Navy Hospital as head of the audiology department. He was employed by the hospital for fourteen years and dispensed over five thousand hearing aids to active duty and retired military. Upon Dr. Wold’s retirement, he was presented with a civil service commendation medal reflecting the large numbers of patient he treated Prior to coming to the Naval Hospital, Dr. Wold was the owner of a private practice speech and hearing clinic in Gulfport, Mississippi. For many years his clinic provided speech and language therapy to adults and children. Hearing aids were dispensed on those individuals needing amplification.
Dr. Wold was a consultant to many head start organizations, to public schools, and to organizations that worked with children with learning and auditory problems. His clinic was the first to introduced computer speech and language therapy to preschool children.
Dr. Wold was an audiological consultant to Mississippi Power company, other heavy and light manufacturing facilities and other companies producing high noise levels. He was instrumental in helping obtain workmen’s benefits for many workers exposed to high noise levels.
Dr. Wold is board certified and licensed in Speech Pathology and Audiology. He is a member of the American Speech and Hearing Association, the American Academy of Audiology, and the Florida Academy of Audiology. He has lectured around the country on hearing and speech problems.
I have been with Dr. Kotlarz since 1999. My goal is to assist him in making your visits flow smoothly. I usually will be the person that brings you from the waiting room to the exam room. Some of my duties include scheduling surgery, answering routine correspondence, such as prescription refills and filing post-surgical leave forms. I prepare the exam room for your visit and assist Dr. Koltarz with office-based procedures. I will coordinate your sleep apnea machine and supplies and to help to coordinate testing such as CT and MRI scans.
I was born in Tacoma, Washington. I have lived in all the four corners of the US. Graduated from Hanover High in New Hampshire, then picked up my toys and headed south to Boca Raton where I received a BS in Education from Florida Atlantic University. I then headed north once again and did some post-graduate work at East Stroudsburg State College in Pennsylvania.
I continued my traveling theme as I headed east … wayyy east to Europe. After living and traveling extensively throughout Europe, including France, England, and Spain, I finally decided to “settled down” in Pensacola. That was some years ago, and I still love the sun and warm weather we have almost all year long.
I was pleased and excited to be allowed to join the Kotlarz team in 2001. I still feel that thrill as I drive to work each day with our only “volunteer” Classic Ladi — my 4yo Westie from Nebraska. I am truly honored to be one of a wonderful team of men and women who genuinely “know and enjoy their chosen professions”.
I was born and raised in upstate New York. I went to college at UWF. Permanently moved to Pensacola in 2003. I enjoy spending time with family, the beach and anything outdoors. I’ve spent many years in retail as a Customer Service Manager before moving into the healthcare field. I am a born organizer and being with the Kotlarz team has been great. I have been here since 2006.
I was born in Roanoke, Virginia but if you ask me, I consider myself a native Pensacolian. I graduated from Pensacola Jr. College in 1979 and went to work for one of the local hospitals.
In 1997 I ventured out into the private office sector with a local group of Otolaryngologists; Dr. Kotlarz came on board in 1998. Once Dr. K opened his own private practice I followed him to continue my billing career. I am a member of the American Academy of Professional Coders the Association of Otolaryngology Administrators; I have held the office of Public Notary for the past eleven years.
I am very involved in the Krewe du YaYa Keeping Abreast Foundations, Inc. which is a nonprofit organization of local women that volunteer their time and efforts to help make a difference in their crusade against breast cancer.
I have four children and have been blessed with four beautiful grandchildren and an additional one on the way. I’m delighted to be a part of the Kotlarz family. I am always available to answer any questions you may have concerning your insurance and billing needs in connection with Kotlarz ENT.
I work behind the scenes dealing with insurance companies for Dr. K. Pre-cert, for those of you not understand that term, is if you need a test or procedure done, I must get it “pre-authorized” first. My day is spent “on the phone” making calls the number on the back of your insurance card. I then have to attempt to convince the representative at the other end of that phone that your particular procedure, surgery or even a test or x-ray is medically necessary. No easy feat I might add is this day of cutbacks and changes. I consider this the most important part of my job here.
Prior to joining the Kotlarz team 7 years ago, I worked for over 20 years in retail; though this is telling my age …. My last job was with the Sports Authority as an Operations Manager. Even farther back then that, I did a similar job with insurance authorizations at the Medical Center Clinic.
I have numerous children and grandchildren that are the light of my life!
